Our Nursing Leadership

Jennifer Gallant, our Founder and CEO, is a registered nurse and healthcare leader with more than 20 years of experience spanning emergency medicine, home health, chronic disease management, care coordination, and population health. Throughout her career, she has worked across the entire continuum of care, helping patients and families navigate complex medical conditions while improving quality of life and healthcare outcomes.

Jennifer began her nursing career in the Emergency Department before transitioning to home-based care management, where she spent five years supporting patients with complex medical and social needs in their homes. She later joined a large healthcare system as a nurse care manager, partnering with primary care providers to reduce hospital utilization and improve patient outcomes. Her success in building strong patient and provider relationships led to rapid advancement into leadership roles.

As a Senior leader of Care Management and Clinical Education, Jennifer built a comprehensive care management department from the ground up, growing the program to more than 20 team members. She developed clinical workflows, educational programs, best practices, and patient-centered care management strategies that continue to improve outcomes for thousands of patients.

Jennifer has a proven track record of reducing emergency department visits, hospital admissions, and readmissions while empowering patients and families through education, advocacy, and personalized support. Through JG Care Partners, she brings her extensive clinical expertise and compassionate approach directly to individuals and families seeking trusted guidance, care coordination, and peace of mind.
